Clinical Scoring Systems and Radiologic Imaging in the Diagnosis of Pediatric Appendicitis.

چکیده
Several clinical scoring systems have been developed to improve diagnostic accuracy in cases of suspected acute appendicitis. In 1986, Alvarado [1] described a scoring system, the Alvarado or MANTRELS score, to aid the diagnosis of acute appendicits. Samuel [2] was the first to describe a Pediatric Appendicitis Score (PAS) specifically for children incorporating eight statistically significant variables: cough/percussion tenderness/hopping tenderness right lower abdomen, anorexia, pyrexia, nausea/ emesis, tenderness in right iliac fossa, leukocytosis, polymorphonuclear leukocytosis, and migration of pain. Assigning a score of 1 to each parameter apart from the physical signs which were assigned a score of 2 each, Samuel [2], in his study of 1170 children, found a sensitivity of 100%, specificity of 92%, positive predictive value (PPV) of 96%, and a negative predictive value (NPV) of 99%. A PAS score of 6 or greater was indicative of a high probability of acute appendicits [2].

Evaluation of Suspected Pediatric Appendicitis with Alvarado Method Using a Computerized Intelligent Model
Background: Acute appendicitis is one of the common and urgent illnesses among children. Children usually are unable to help the physicians completely due to weakness in describing the medical history. Moreover, acute appendicitis overlaps with conditions of other diseases in terms of Symptoms and signs in the first hours of presentation. These conditions lead to unwanted biases as well as err...
متن کاملThe role of 99mTc-Ubiquicidin (UBI) and 99mTc-IgG scintigraphies in diagnosis of acute appendicitis: A preliminary result
Introduction: Appendicitis is one of the most common surgical emergencies. In spite of the relatively high rate of negative appendectomy, as a result of miss diagnosis, uncertainty of diagnosis still continues to challenge physicians. The objective of this prospective study was to investigate the role of 99mTc-Ubiquicidin (UBI) scintigraphy in the diagnosis of acute appendicitis and ...

Introduction: Appendicitis is one of the most prevalent causes of abdominal pain requiring surgery. Diagnosis of appendicitis is one of the important challenges of surgeons in emergency care. Delay in the diagnosis of appendicitis is associated with an increase in disability and mortality. The timely and correct diagnosis of appendicitis is necessary to reduce the complications of appendicitis ...
متن کاملEvaluation of Children's Appendicitis Score Compared With Pediatric Appendicitis Score in Diagnosis of Pediatric Appendicitis
Introduction: Identification and promotion of diagnostic methods has been a continuous effort to reduce disease and its complications and reduce costs associated with treatment. Despite all these efforts and improving our knowledge of diseases and diagnostic tools, pediatric appendicitis remains part of the diagnostic challenge in the surgical field. The aim of this is to compare the two diagno...
متن کاملNeonatal Appendicitis: Difficult Diagnosis with Enterocolitis
The clinical presentation of appendicitis in the newborn is nonspecific. The diagnosis was always made after surgical exploration for acute abdominal findings mimicking necrotizing enterocolitis. We report the case of acute appendicitis in a newborn diagnosed intraoperatively after neonatal occlusion symptoms.
